NEW PATIENT REGISTRATION
If you are a new patient you can register your details online before your first visit. Once this form has been submitted one of our administration staff may contact you to confirm your details or to clarify a point, prior to your fist visit. To access this form please click on the link below.
Your First Visit
Please bring your referral with you from your general practitioner. It is also important to bring any medical imaging such as X-rays, CT, MRI, ultrasounds and bone scans. Dr Hartley might organise further investigations and ask you to return for a second appointment.
Consultation CostS
1st consultation: $200 ($75.05 back from Medicare)
2nd and further pre-operative consultations: $100 ($37.30 back from Medicare)
Post-operative consultation: Free
Surgical Fees
The costs of surgery in Australia are complicated and depend on many variables including the type of condition being treated, its severity, the type of operation being performed, what is done during surgery as well as other variables such as which health fund you belong to, whether you are covered by DVA or WorkCover and sometimes to which hospital you are admitted.
For most operations for most patients (depending on your health fund) Dr Hartley charges either AMA rates or a “known gap”. Some operations might be charged above AMA rates and some might be “no gap” (free).
